Regardless of the job you're advertising or the size of your business, you should always have a formal application form as part of your recruitment process. An up-to-date job application form ensures you use a common set of information to make the right selection. 
 
Use this sample application form as a template to draft an application form that meets the needs of your business.
 
You may notice that the job application form template does not ask questions regarding race, religion, ethnic origin, sex, belief, marital status, sexual orientation, dependants/children/child-minding arrangements, or age; this is because such questions are discriminatory under the Equality Act.
